In order to provide you with the best maternity care and streamlined service, we’ve posted a variety of forms on our Web site, including new patient registration, prenatal history, general pregnancy information, over-the-counter drug information, release of medical records and disability forms. Upon your first visit to our office, please bring with you the completed patient registration form and prenatal history (if applicable).

Your medical records are strictly confidential. Your permission is required before we will release information about your care. We must have a release signed by you in order to release your records to another party. After you authorize us to release information from our records, it may take up to two weeks to process your request.
